We are a private-pay, concierge physical therapy practice. Insurance is not billed. Payment is due at the time of service, and detailed receipts are provided for patients who wish to submit for possible out-of-network reimbursement.

Please complete all required intake paperwork prior to your first visit and provide thorough, accurate information about your history and concerns. This allows your evaluation to be efficient and ensures your appointment time is dedicated to assessment and treatment—not paperwork.
